A broken or loose bracket is one of the most common things that happens during orthodontic treatment, and it happens to careful patients too. In most cases it is not an emergency. But do not pull it off, do not ignore it, and do call your clinic so the team can decide whether you need to be seen this week or at your next scheduled visit. What you do in the first few hours decides whether this is a small delay or a painful few days.

What should you do in the first hour?

Look, do not pull. Check whether the bracket has come off the tooth completely or is still threaded onto the wire. A bracket sliding on the wire is annoying but stable. Pulling at it can bend the wire and move your teeth in the wrong direction.
If it is still on the wire, leave it in place. It is safer there than loose in your mouth.

What if you swallowed the bracket?

Swallowing a small bracket happens and is usually harmless. It typically passes through the digestive system without causing any problem, and you do not need to go looking for it.

Seek urgent medical attention, though, if you have any of these after swallowing a piece of your appliance: coughing that will not settle, wheezing or difficulty breathing, chest pain, severe abdominal pain, or vomiting. Those can suggest a piece went into the airway rather than the stomach, or is causing a problem on the way down. This is a hospital matter rather than a dental one.

What about a wire poking your cheek?

A wire that has slid to one side, or an end that has worked its way out of the tube at the back, is the other common problem, and it is the one that actually hurts.

Cover the end with orthodontic wax. This is the main fix until you are seen.
If the wire is long and bendable, you can gently push it flat against the tooth with the rubber end of a pencil or a clean cotton bud.
Rinse with warm salt water a few times a day if the cheek is already ulcerated.
Do not cut the wire yourself unless your orthodontist has told you to. Cut ends can be sharper than what you started with, and a cut piece can be inhaled or swallowed.
Do not use pliers, nail clippers or improvised tools from the toolbox on any part of your appliance.

If the wire is embedded in the cheek or gum, or you cannot cover it and cannot eat or sleep, that is an urgent visit. Say so when you call.

Is a broken bracket an emergency?

Usually no. Call the clinic the same day and let them decide the timing. Treat it as urgent, and say so clearly when you contact us, if:

A wire is stuck into the cheek, lip, tongue or gum
There is swelling of the face or gum, or you have a fever
You have had a blow to the mouth and a tooth is loose, chipped or has moved
There is bleeding that does not stop
Pain is severe rather than the dull ache of ordinary tooth movement

Everything else, including a loose bracket, a lost ligature, a broken band or a detached retainer, is an "arrange an appointment" problem rather than a "rush somewhere tonight" problem.

Will a broken bracket make treatment take longer?

It can. A bracket that is off or loose is no longer guiding that tooth, so that tooth pauses while the others keep moving. One bracket rebonded promptly usually costs nothing more than the time it takes to fix. Repeated breakages, or a bracket left off for weeks, are the main reason treatment overruns the estimate you were given at the start. Getting it repaired quickly is the cheapest thing you can do for your timeline.

Why do brackets break?

Hard foods such as nuts, hard sweets, ice, crusts, roasted maize, sugarcane and bones
Sticky foods that pull at the bracket, including toffees, chewing gum and sticky sweets
Biting whole foods with the front teeth: apples, carrots, corn on the cob, mandazi with a firm crust. Cut them into pieces instead.
Nail biting, pen chewing and opening things with your teeth
A knock to the mouth during sport, which is why a mouthguard is worth wearing
Sometimes the bond simply fails, especially on teeth with fillings, crowns or unusual enamel. This is nobody's fault.

Is it normal for new braces to hurt for the first week?

Yes. For the first three to seven days after braces are fitted or adjusted, expect general soreness, tenderness when biting, and cheeks and lips that feel rubbed raw while they toughen up. That is normal tooth movement and it settles. What helps:

Soft food for the first few days: soup, ugali, mashed potato, eggs, yoghurt, rice, well-cooked vegetables
Orthodontic wax over any bracket that is rubbing
Warm salt-water rinses for ulcers
Ordinary over-the-counter pain relief you already use safely, taken as directed on the packet
A soft toothbrush and patient brushing, because sore gums still need cleaning and skipping it makes everything worse

Sharp, localised, worsening pain in one tooth is different from general soreness. Have that checked.

Can you still clean your teeth properly with a broken bracket?

Yes, and you must. Keep brushing gently around the loose bracket rather than avoiding the area, because plaque left around brackets causes the white marks and decay that show up when braces come off. If flossing is impossible around the damaged area, use an interdental brush or a water flosser until it is repaired, and mention any bleeding gums at your appointment.
