Why Is My Tongue White? Coating, Thrush and Patches That Need a Look

Look at your tongue first thing in the morning and it is probably whiter than it was last night. That alone is not a problem.

The question that matters is a simple one, and you can answer it at home: does the white come off?

Why does my tongue look white?

The top of the tongue is not smooth. It is covered in tiny projections, a bit like the pile on a carpet. Dead cells, bacteria and bits of food collect between them, and when enough builds up the tongue looks white or creamy, usually most at the back.

That build-up gets worse when your mouth is dry. Sleeping with your mouth open, not drinking much water, a fever, a cold that has you breathing through your mouth, smoking, and days of eating only soft food all thicken the coating, because normal chewing and saliva are what usually rub it away.

How do I get rid of a white coating?

Clean it, gently, once or twice a day.

A tongue scraper works well. So does the back of your toothbrush. Reach as far back as you comfortably can and draw it forward a few times, rinsing in between. You are aiming to remove the coating, not scrub the tongue raw.

Drink water through the day. Treat whatever is drying your mouth, if you can.

A coating that clears with this is a coating. It also tends to be behind a good share of bad breath, which is covered at celestialdental.org/bad-breath-even-after-brushing-causes-and-fixes.

What if the white patches wipe off but leave it red and sore?

That sounds more like oral thrush.

Thrush is an overgrowth of a yeast, candida, that lives harmlessly in most mouths until something tips the balance. It looks like creamy white patches, sometimes like cottage cheese, on the tongue, the inside of the cheeks or the roof of the mouth. Wipe them and the surface underneath is red and can bleed slightly. It may burn, and food can taste odd.

The common triggers are worth knowing, because they point to the fix. A course of antibiotics. Steroid inhalers for asthma, especially if you do not rinse after using them. Dentures left in overnight. Diabetes, particularly when sugar levels are high. A very dry mouth. Babies and older people get it more easily, and so does anyone whose immunity is lowered.

Thrush needs an antifungal treatment, which a doctor, pharmacist or dentist can advise on. Scraping alone will not clear it.

If you use an inhaler, rinse your mouth with water and spit after every dose. If you wear dentures, take them out at night and clean them. And if thrush keeps coming back with no obvious reason, please have your blood sugar checked. Recurrent thrush is one of the ways diabetes first shows itself.

What does it mean if the white patch will not wipe off?

That is the one to have examined.

A firm white patch that stays put, especially on the side of the tongue or the floor of the mouth, may be leukoplakia. That is a descriptive name, meaning a white patch with no obvious cause. Many are harmless. Some are changes in the lining of the mouth that can, over years, become cancer, which is why we do not guess about them.

They are much more common in people who smoke, chew tobacco or drink heavily.

Get a white patch that does not wipe off looked at if it has been there more than two weeks, and sooner if it is thickened, rough, has red areas mixed in, is sore, or sits next to an ulcer that will not heal. An examination is quick. Sometimes we will recommend a small sample to be sure. That is caution, not a diagnosis.

My tongue has smooth red patches with white edges that move around. Is that serious?

That is very likely geographic tongue, and no, it is not.

The patches look like a map, with smooth red areas outlined by a pale, slightly raised border. The odd thing is that they change shape and position over days, as if the map is being redrawn. Some people never notice it. Others feel stinging with spicy food, citrus or very hot drinks.

It is not an infection, it is not contagious, and it is not linked to cancer. It tends to come and go for years. If it stings, avoiding the foods that trigger it during a flare is usually all it needs.

What about white lines on the cheeks or tongue?

Fine, lacy white lines, often on both cheeks, are typical of a condition called oral lichen planus. It is not contagious, often causes no symptoms, and sometimes becomes sore. It is worth having diagnosed and kept an eye on at your check-ups.

My baby's tongue is white. Is that milk or thrush?

Milk residue sits on the tongue only and wipes off easily with a soft damp cloth.

Thrush in babies spreads to the inside of the cheeks and lips, does not wipe off easily, and can make feeding uncomfortable. A breastfeeding mother may notice sore, red nipples at the same time. That combination needs treatment for both baby and mother, so see your doctor or clinic.

When should I see someone about my tongue?

When a white patch does not wipe off. When it has lasted more than two weeks. When it is sore, bleeding or lumpy, or there is an ulcer that is not healing. When thrush keeps returning. And whenever your instinct says something has changed.

The same two-week rule applies to ulcers, which are covered at celestialdental.org/mouth-ulcer-that-wont-heal-causes-and-when-to-worry.
