Nobody warns you properly about the first week. The braces go on, the appointment itself is painless, and then a few hours later your teeth ache, biting feels strange, and the inside of your lips is raw. It is the point where a fair number of people quietly decide orthodontics was a mistake.

It settles. Knowing the pattern, and having the right things in the house before you need them, makes the difference between a rough few days and a miserable fortnight.

Why do braces hurt?

Not because they are tight against the tooth, but because they are working. A brace applies gentle continuous pressure, and the bone around each root remodels to let the tooth move. That remodelling involves inflammation, and inflamed ligament around a tooth root is tender, particularly when you bite.

There are two separate sources of discomfort, and they need different remedies:

Aching teeth, from the pressure and the movement. Worst on days one to three, then fading
Soft-tissue soreness, where brackets and wire ends rub the inside of the lips, cheeks and tongue until the tissue toughens

Aligners follow the same pattern with each new tray, usually milder, without the rubbing.

What helps the rubbing and the ulcers?

This is the part most people underestimate, and it is the most fixable.

Orthodontic wax. Dry the bracket with a tissue, roll a pea of wax, press it over the offending bracket. Your orthodontist gives you some at fitting. Keep a spare packet in your bag
Silicone covers or wax over the sharpest brackets, usually the canines, which sit right where the lip folds
Salt water rinses several times a day
A soft toothbrush and gentle brushing along the gumline, since sore gums get neglected and then bleed
Time. The inside of the cheek toughens within one to two weeks and stops being a problem

If a wire end is poking, wax it, and if it is long, it can be tucked or trimmed at the practice. Do not cut wires yourself with household pliers.

What can I eat in the first week?

Soft, cool and not sticky. Shop for week one before the braces go on, because deciding what to eat while your teeth ache goes badly.

What works well from an ordinary Kenyan kitchen:

Uji, cooled to warm rather than hot, which is the easiest thing to get down on day one
Mala or yoghurt, cold from the fridge, which soothes as well as feeds
Mashed potato, mukimo, mashed matoke or mashed sweet potato
Beans or njahi cooked soft and mashed, rather than whole and firm
Soft ugali with sukuma or spinach cooked down well, not stiff ugali torn and chewed
Scrambled or boiled eggs, avocado, ripe banana, pawpaw, mango without the fibre
Soup, broth and well-cooked rice, fish, or minced meat in a stew
Smoothies, which are useful on the two worst days, taken with a spoon rather than sucked hard through a straw

What to keep away from, in week one and for the whole treatment:

Nyama choma and any meat off the bone. This is the most common reason we see a bracket off in the first month
Mahindi choma, sugarcane and roasted or raw maize. Biting maize off the cob and chewing sugarcane will take brackets off almost reliably
Hard mandazi or crusty bread, crisps, groundnuts, kashata and hard sweets
Chapati, which is not hard but is chewier than people expect and pulls at sore teeth in the first few days
Sticky things: toffee, sugary chewing gum, sweets that pull
Whole hard fruit and raw carrots, unless cut small
Biting with the front teeth. Cut apples, meat and sandwiches up and chew at the back
Sodas, sweet chai and frequent snacking, which cause the permanent white decay marks around each bracket that only become visible on the day the braces come off

Hot food and drink is fine, but very hot things feel worse while the teeth are tender.

Where do I get wax and painkillers here?

Nothing on the relief list is hard to find, and none of it needs a prescription.

Orthodontic wax comes from us. You are given a packet at fitting, and you can collect more from any branch at no cost, so ask rather than suffering with a bracket that is cutting your lip. In an emergency at night, a small piece of clean sugar-free chewing gum, chewed until soft and pressed over the bracket, will hold until morning
Paracetamol or ibuprofen is stocked by every chemist. Buy a packet on the way home from the fitting rather than at eleven at night
Salt for rinses you already have. Half a teaspoon in a glass of warm water
A soft brush and an interdental brush for cleaning around the brackets, from any pharmacy or supermarket

One piece of scheduling advice specific to how far people travel to us. If your adjustment appointment means a long matatu ride home, take your painkiller before you leave the clinic rather than when you arrive, since the ache builds over the first few hours and you do not want it peaking while you are still on the road. Will it hurt every time?

Less each time. The first fitting is the worst appointment of the whole treatment. Adjustments are typically one to three days of mild ache. Some stages, such as when space closing begins or when a stubborn tooth is being rotated, are more noticeable than others.

Getting a new aligner tray is similar and usually milder. Changing trays at night is a common trick, so the tightest hours pass while you sleep.

How do I keep my teeth clean while they hurt?

Carefully, and without skipping. This is when decay damage starts, because sore gums make people brush timidly for a week and plaque builds around every bracket.

Does insurance or SHA cover braces?

Usually not, or only partially. Orthodontic treatment is normally classed as elective, and most standard plans and SHA cover focus on routine, preventive or medically necessary care, so braces, whitening and other cosmetic work are commonly excluded. Some enhanced schemes include an orthodontic benefit, often capped.
